Biomechanical Mapping of the Female Pelvic Floor Excerpt :

The female pelvic floor includes the muscles, ligaments, connective tissues and nerves that support the uterus, bladder, vagina and rectum. It is a complex system which may have multiple diseased conditions related to changes in biomechanical conditions of pelvic tissues and structures. Among them are highly prevalent pelvic organ prolapse, urinary incontinence and tissue atrophy. Biomechanical Mapping of the Female Pelvic Floor explores new technological advances in women's healthcare intended to improve pelvic floor characterization, diagnosis and prediction of treatment outcomes. By discussing different technologies used on the biomechanical characterization of pelvic floor, it describes the anatomy, physiology, musculoskeletal, and tissue characteristics of the pelvic floor tissues, and explores how to detect conditions that may lead to problems such as spontaneous preterm delivery and childbirth trauma, providing insight into ways to reduce childbirth related injury and Pelvic floor disorders (PFDs). This book is a valuable resource for researchers focused on gynecology, urogynecology or obstetrics, clinicians, graduate students and biomedical scientists and bioengineers who need to understand more the technological advances on biomechanical characterization and how they can be used not only for diagnosis but also for monitoring several OBGYN-related conditions.

The Female Pelvic Floor Excerpt :

The initial objective of this work was to reduce stress incontinence surgery from a major surgical procedure (requiring up to ten days in hospital) to a minor day-care operation. From the beginning it was clear that the two major impediments to achieving this goal were post operative pain and urinary retention. Addressing these problems became a long and winding road and culminated in the Integral Theory. The IVS 'tension-free' tape operation was inspired by Dr Robert Zacharin's anatomical studies. Though Zacharin suggested that the ligaments and muscles around the urethra were important for urinary continence control, he did not say how. The observation that implanted foreign materials created scar tissue led to the hypothesis that a plastic tape inserted in the position of the pubourethrallig ament, would leave behind sufficient scar tissue to reinforce that ligament, which would then anchor the muscles for urethral closure. In September 1986, two prototype Intravaginal Sling operations were per formed. A Mersilene tape was inserted with neither tension nor elevation, in the position of the pubourethral ligament. Restoration of continence was immediate and both patients were discharged on the day following surgery without require ment for catheterization. There was minimal pain, and immediate restoration of continence. After six weeks the tapes were removed. Both patients were still conti nent at last review 10 years later. The results appeared to confirm the importance of a midurethral anchoring point.

This fourth edition has changed fundamentally in presentation and content to provide the clinician with the evidence and clinical tools for effective practice. The new model presented in this edition - The Integrated Systems Model and the Clinical Puzzle - co-developed by Diane Lee & Linda-Joy Lee, facilitates effective clinical reasoning, hypothesis development and prescriptive treatment. It is highly unlikely that there will ever be enough research evidence to me the needs of a clinician who is faced with patients presenting with a wide and variable range of single and multiple impairments every day. Clinical expertise (knowing how to do the right thing at the right time) comes from disciplined, reflective practice and it is hoped that this text will help more clinicians become expert in this field. Presents an evidence-based approach to the examination, diagnosis and treatment of the lumbopelvic region Easy to read and clinican friendly Demonstrates how clinicians can translate knowledge derived from scientific research into clinical practice and also use knowledge gained from clinical practice to evaluate the relevance of the scientific research Highly illustrated descriptions of tests and techniques for practice The author team - Diane Lee, Linda-Joy Lee and Andry Vleeming - all have international reputations as clinicians and researchers Book now available in full colour online! Pelvic Ring Fractures Excerpt :

This book provides in-depth coverage of all aspects of pelvic ring fractures and their management. The opening chapters supply essential information on surgical anatomy, biomechanics, classification, clinical evaluation, radiological diagnostics, and emergency and acute management. The various operative techniques, including navigation techniques, that have been established and standardized over the past two decades are then presented in a step-by-step approach. Readers will find guidance on surgical indications, choice of approaches, reduction and fixation strategies, complication management, and optimization of long-term results. Specific treatment concepts are described for age-specific fractures, including pediatric and geriatric injuries, and secondary reconstructions. Pelvic ring fractures represent challenging injuries, especially when they present with concomitant hemodynamic instability. This book will help trauma and orthopaedic surgeons at all levels of experience to achieve the primary treatment aim of anatomic restoration of the bony pelvis to preserve biomechanical stability and avoid malunion with resulting clinical impairments.
