Total joint arthroplasty is an effective surgical procedure for end-stage osteoarthritis of major joints with satisfactory long term clinical outcome. A large and growing number of arthroplasties are performed annually worldwide and a great number of orthopaedic surgeons are practicing arthroplasty surgery as their main surgical activity. The biological behavior of the bone-implant interface is crucial for the long term survival of the artificial joint. All factors which have a positive or negative effect on the interface are of great interest for those practicing arthroplasty surgery. Basic scientists and the industry are continuously searching for new implant fixation mechanisms and improved materials. There is an accumulation of a great amount of basic science data (both biological, material and mechanical) related to the incorporation or loosening of the bone-implant interface. However, basic science data does not always translate to satisfactory clinical application, and orthopaedic practitioners often wonder which piece of information is clinically useful. A further problem is that basic scientists often speak their own scientific language and may not fully appreciate common clinical practice needs. In this textbook the biological and mechanical mechanisms of implant incorporation and loosening will be presented. All new data concerning materials and methods for incorporation enhancement will be critically analyzed. Data useful for clinical application will be stressed. All these, and other problems related to the implantation of the devices, the surgical procedures, and device pathology, were. discussed in light of current, as well as, emerging technologies and scientific knowledge. Repeatedly, scientists designing prostheses became aware of a lack of understanding of physiological phenomena associated with biocompatibility; the interchange among practising physicians, basic scientists, and pathologists at this Institute was appreciated. We thank all the contributors and participants for their effort. Thanks are also due to the personnel of the Scientific Affairs Division of NATO. Abstract: Aseptic loosening of metallic implants is a common complication of orthopedic surgery. The bone-implant interface is frequently the "weak link" in orthopedic constructs. Since the beginnings of orthopedic surgery, attempts have been made to increase the bonding strength between bone and metallic implants. Although advancement has been made, the bone-implant interface remains the most common point of failure for long-term use of implants. We hypothesized that osseointegration could be increased if a coating of bone morphogenetic protein-2 (BMP-2), a protein that has been shown to stimulate new bone growth, was applied to a metal implant. We also hypothesized that polycaprolactone (PCL), a bioresorbable polymer, could be used as a vehicle to deliver BMP-2 to the implant surface. The first phase of this project was a pilot study to determine the in vitro elution of the BMP-2 from PCL-coated implants. The second phase of the project evaluated biocompatibility and coating integrity for implantation in bone. This data was used to modify features of the coating and the bone implantation site. These changes were implicated in phase 3 of the study. Phase 2 and 3 involved implantation of PCL / BMP-2 coated screws using a sheep model to evaluate the osseointegration. Peak removal torque of screws was measured and response of the bone in the area of the screw was determined using radiographs, bone mineral density, and histomorphometry. The physiologic response to the implants was assessed by measuring serum bone specific alkaline phosphatase and osteonal activity in the bone around the screws. The biocompatibility to the PCL and the PCL/BMP-2 was also analyzed. This was assessed by looking histologically at PCL and PCL/BMP-2 implants in subcutaneous pockets. Only subtle differences among the treatment groups were found.
