Chronic Headaches Excerpt :

This is the first new book in many years to provide a comprehensive review of the latest theory, research, and treatment of chronic headaches from a biopsychological perspective. It is designed to make the tools of assessment and therapy widely accessible, while placing them in the context of how the disorders arise. The physiology and psychology of pain, and each disorder, are reviewed in an accessible manner. Clinical experience, laboratory data, and illustrative vignettes aid in treatment selection. Part I introduces the major types of headaches and provides a comprehensive review of pain. Part II details the major forms of headaches—migraine, tension-type, cluster, secondary, and headaches in children. A clinical presentation introduces each type of headache, followed by the physiological and psychological underpinnings and their implications for assessing and treating patients. Part III serves as a clinical guide for practitioners. The book closes with an analysis of how well the treatments work, the mechanisms behind the efficacy, and guidelines for treatment matching. A range of practical tools is incorporated. Clinical evaluation is reviewed in depth, including the interview, psychometrics, and psychophysiological assessment. Key sections of the 2004 International Headache Society diagnostic criteria serve as a “mini” diagnostic manual. Tables allow rapid look-up of the various disorders and their distinguishing characteristics; trigger point referral patterns; and the comparative merits of migraine medications. Blank headache diaries, appropriate for various stages of treatment, serve as models. A relaxation exercise is provided, as are muscle tension and hand temperature norms. Key terms are defined in the extensive glossary to help psychologists and medical professionals share vocabulary. Medical, herbal, and behavioral therapies are discussed in terms of the underlying science. Chronic Headaches is intended for healthcare providers, pain specialist

Headache Medicine Excerpt :

Headache is the number one complaint that causes a patient to see a neurologist. Because headaches constitute such a large part of many clinicians practices, the United Council for Neurologic Subspecialties created an annually-administered headache subspecialty exam in 2006. Headache Medicine: Questions and Answers is the only question-and-answer book for the new headache subspecialty exam, with more than 500 questions, answers, explanations, and references to help readers self-assess their knowledge and to prepare for the subspecialty exam, as well as the neurology Board examination. Headache Medicine: Questions and Answers covers everything from the basics of epidemiology and co-morbidity, to must-know clinical neurology, to diagnosis and treatment. Board-type review questions are used throughout, each with an answer, a detailed explanation, and one or more references to help direct more in-depth review as desired. Divided into six key sections for targeted topic review, this is the most comprehensive and effective way to prepare for exams. Key features of Headache Medicine include: Board-type question format used to provide familiarity with question types Answers and explanations, which detail not only why the answer is correct, but also why the distracters are wrong. A series of in-depth, multi-question patient management problems that provide a realistic clinical learning experience References with each question for further investigation as desired

Comprehensive Review of Headache Medicine Excerpt :

Headache Medicine has recently become an official medical subspecialty. Practitioners who specialize in the field come from many different backgrounds including neurology, internal medicine, anesthesiology, rehabilitation medicine, family medicine, psychiatry, dentistry and others. A great deal of research is being done in HM, and, as a result, the field is one which is growing rapidly in every way. Interestingly, most patients with headache are not being treated by specialists in HM, in large part because of the vast numbers of patients requiring medical attention for headache disorders, and the relatively small number of bona fide specialists. Thus, there is a need for resources to guide these practitioners as they manage headache patients. There are several excellent scholarly texts in the field and numerous books for the public about headache. There are really no concise manuals of HM, however, there are books aimed at the bulk of skilled practitioners taking care of the major share of headache sufferers, who wish to understand the important concepts of diagnosis, treatment, and prognosis in this complex field. This is the aim of Dr. Levin's book which consists of 4 parts: Part 1- Headache Medicine Basic Science, Part 2- Diagnosis of Primary Headache Disorders, Part 3- Diagnosis of Secondary Headache Disorders, and Part 4- Headache Treatment. Chapters in each section are concise, but include cutting edge information about these aspects of headache medicine. The organization of the contents of this book is similar to the Curriculum for Headache Medicine designed by the United Council for Neurologic Subspecialties (UCNS) which is the organization responsible for accrediting Headache Medicine training programs and accrediting individual practitioners. The content of the book mirrors that of the UCNS board examination as well. Chapters include key tables and illustrations. Review questions are included at the end of each chapter and serve as a study resource. The ed

Drug Induced Headache Excerpt :

M. WILKINSON Patients with frequent or daily headaches pose a very difficult problem for the physician who has to treat them, particularly as many patients think that there should be a medicine or medicines which give them instant relief. In the search for the compound which would meet this very natural desire, many drugs have been manufactured and the temptation for the physician is either to increase the dose of a drug which seems to be, at any rate, partially effective, or to add one or more drugs to those which the patient is already taking. Although there have been some references to the dangers of overdosage of drugs for migraine in the past, it was not until relatively recently that it was recognized that drugs given for the relief of headache, if taken injudiciously, may themselves cause headache. The first drugs to be implicated in this way were ergotamine and phenazone. In the case of ergotamine tartrate, the dangers of ergotism were well known as this was a disorder which had been known and written about for many years. In the treatment of headache, fully blown ergotism is rare and in recent years has usually been due to self-medication in doses much greater than those prescribed although there are a few recorded cases where toxic amounts have been given.

Pathophysiology of Headaches Excerpt :

This book provides a detailed overview of the current state of knowledge regarding the pathophysiology of both primary headaches – migraine, tension-type headache (TTH), and cluster headache – and the very important and frequent type of secondary headache, medication overuse headache (MOH). After an introductory chapter describing relevant neuroanatomy and vascular anatomy, the evidence gained from animal models regarding the pathophysiology of migraine and the other primary headaches is reviewed. Knowledge of the genetic component in the different types of headache is then examined with reference to recent evidence, for example regarding the implication of the trigeminovascular system and cortical spreading depression in migraine. Detailed information is provided on insights into primary headaches from imaging studies, including functional magnetic resonance imaging and positron emission tomography and on their neurophysiology and biochemistry. A further series of important chapters describe present knowledge of the pathophysiology of each specific type of headache and consider future directions. Written by acknowledged experts in their fields from Europe and the United States, clinicians and students will find Pathophysiology of Headaches to be an excellent source of up-to-date information on why patients experience headaches. In addition, it will be of value for pain researchers investigating the underlying mechanisms of headache.

Migraine Excerpt :

In recent years the bestselling Awakenings and The Man Who Mistook His Wife for a Hat have received great critical acclaim, but Oliver Sacks's readers may remember that he began his medical career working with migraine patients. In this, the latest edition of "Migraine," he returns to his first book and enriches it with additional case histories, new findings, and practical information on treatment. To define "migraine, " suggests Oliver Sacks, one must embrace the dizzying variety of experiences of its sufferers. For some, the affliction features of a headache of monumental proportions. For others, there is no pain at all. Some attacks are triggered by weather, some intense light. Still others consist of intense light -- hallucinatory displays of dazzling loops, stars, and geometrics. "Migraine" is Sacks's brilliant examination of a debilitating ailment and the profound implications of neurological illness. Synthesizing his patients' case histories with 2,000 years of human research into the problem, he casts the migraine as exemplar of our psychological transparency, a complex biological response to external factors. Here is a classic meditation on the nature of health and malady, on the unity of mind and body. Here, too, is Sacks's discovery of how the migraine shows us, through hallucinatory displays, the elemental activity of the cerebral cortex -- and potentially, the self-organizing patterns of Nature itself. Enormously compelling, compassionate, and profound, Migraine offers comfort for sufferers -- and insight to all.
