This unique book discusses the management of neurocritical care patients, including basic concepts, pathophysiologic principles, monitoring, treatment indications, and factors that affect outcomes in patients requiring neurocritical care assistance. It addresses the need to improve continuing education in this area, highlighting patient care in the perioperative period. This is the first book to provide a simplified overview for neurosurgeons and neurologists to understand the neurocritical patient journey. It is divided into three parts: the first covers the basics concepts, from monitoring to the interpretation of exams; the second explores general management of specific situations encountered in intensive care and the last part includes prognostic and rehabilitation models, as well as new perspectives. Thanks to the accessible, neurosurgical specific language, the book is well suited for all professionals involved in neurocritical care, including students, but is also a valuable resource for residents and researches, as well as experienced neurosurgeons or neurologists looking for updated information and guidelines.

An authoritative and comprehensive review of the most important clinical issues facing critically ill neurologic and neurosurgical patients. The authors provide pertinent basic, clinical, diagnostic, and management guidelines for all the conditions commonly encountered in the neurocritical care unit, including the management of autonomic disorders that require critical care, postoperative management, and endovascular treatment. The authors also discuss the latest developments in the monitoring of different body systems, emphasizing the management of cardiorespiratory complications and other medical conditions that may threaten the patient's life. Also detailed are the concepts of intracranial physiology and current neuromonitoring techniques, subjects whose understanding is basic to effective management of critically ill neurologic patients. Numerous tables, figures, diagrams, and radiographs simplify and explicate both the diagnostic and therapeutic processes.

An authoritative pocket-sized guide on the medical and surgical management of neurosurgical intensive care patients Decompression is still the mainstay of surgical intervention for neurosurgeons treating neurocritical care patients. However, during the last 20 years, an evolution away from a mechanistic approach has transformed neurocritical care into an increasingly multidisciplinary field. Neurosurgical Intensive Care, 2nd Edition reflects this new paradigm, authored by a neurosurgeon with contributions from experts in the fields of neurology, vascular neurology, interventional neuroradiology, anesthesiology, critical care, traumatology, nutrition, and advanced practice nursing. Neuromonitoring advances have enabled customized interventions tailored to each patient's unique circumstances. The critical care of neurosurgical patients has evolved from an emphasis on pulmonary care (ventilation and oxygenation) to a more nuanced understanding of cerebral protection measures required to manage a disrupted blood-brain-barrier. This edition encompasses advances in the use of ICP monitors, external ventricular devices, brain tissue oxygen monitoring devices, cerebral microdialysis, fluid and electrolyte correction, as well as the increasing use of continuous EEG in the ICU. Key Features Straightforward summaries for each stage of patient care including bedside exams, neuroimaging, lab work, triage procedures, sedation and pain management, neuropharmacology, nutritional needs, administration of fluids, and more. 159 tables and figures facilitate rapid evaluation and decision making. Clinical pearls on a wide range of ICU scenarios - from pathophysiology and management of coma - to interventions for spinal cord injury, multisystem injuries, acute ischemic stroke, and pediatric cases. Insights on how to handle family communication and spiritual needs, medical-legal issues, and discharge planning This compact, highly practical handbook provides a stellar reference for managin

Brain injury is a worldwide leading cause of mortality and morbidity and requires early and appropriate management to minimize these adverse sequelae. Despite such needs, access to specialist centers is limited, forcing both immediate and secondary care of these patients onto generalist staff. These responsibilities are made more problematical by differences in patient management between and even within specialist centers, due in part to an insuffcient evidence-base for many interventions directed at brain injury. This book is borne out of the above observations and is targeted at em- gency and acute medicine, anesthetic and general intensive care staff caring for brain injury of diverse etiology, or surgical teams responsible for the inpatient care of minor to moderate head trauma. Although explaining the various facets of specialist care, the book is not intended to compete with texts directed at neurosciences staff, but aims to advise on optimal care in general hospitals, including criteria for transfer, by a combination of narrative on pathophysiology, principles of care, templates for documentation, and highly specifc algorithms for particular problems. It is intended that the content and structure can form the basis of guidelines and protocols that refect the needs of individual units and that can be constantly refned. Our ultimate goal is to promote informed, consistent, auditable, multidisciplinary care for this cohort of patients and we hope that this text contributes to that process.

The current practice of medicine is largely moving toward applying an evidence-based approach. Evidence-based medicine is the integration of best research evidence using systematic reviews of the medical literature and then translating it into practice by selecting treatment options for specific cases based on the best research. Clinicians rely on the availability of evidence and accordingly take decisions to provide best treatment to their patients. Clinical management of neurologically compromised patients is challenging and varied; for this reason, treating physicians including neuroanesthesiologists are always in search of best available evidence for patient management and care. Essentials of Evidence-Based Practice of Neuroanesthesia and Neurocritical Care highlights the various controversies that exist in the practice of neuroanesthesia and provides conclusive evidence-based solutions. This comprehensive resource succinctly discusses evidence-based practice of neuroanesthesia based on systematic reviews in clinical neuroscience research. Topics include neurophysiology: ICP or CPP thresholds; neuropharmacology: intravenous or inhalational anesthetics; and neuromonitoring: ICP monitoring. Evidence-based practice is now an integral part of neuroscience, and this book will help residents and trainees gain knowledge to apply it to their practice. • Highlights the various controversies that exist in the practice of neuroanesthesia and provides conclusive evidence-based solutions • Topics include neurophysiology: ICP or CPP thresholds; neuropharmacology: intravenous or inhalational anesthetics; and neuromonitoring: ICP monitoring • Provides residents and trainees with the knowledge to apply evidence-based practice of neuroanesthesia to their practice

Written by neurosurgery residents for neurosurgery residents, The Neurosurgical Consult Book focuses on the initial care and treatment of the neurosurgical consult patient, providing the key guidance you need when quick clinical management decisions are critical. You’ll learn to recognize the acuity of patient presentation and determine how to immediately triage the needs of the patient at an appropriate level of care. This portable, practical guide helps you quickly steer neurosurgical patients to medical stability and safety until a definitive clinical management plan is determined. Features a real-life approach with chapters titled by the symptoms and clinical presentation of the patient, not by neurosurgical diagnosis. Begins each chapter with the actual consult notification from the consulting team, mimicking how the neurosurgery provider is notified about a new patient in practice. Presents initial imaging as it occurs in real life—whether imaging has already been done or whether the consulting physician needs guidance on what imaging is needed. Includes Walking Thoughts, guidance on how to tailor your thoughts toward the most important and pressing issues facing the patient before even meeting them, so that upon seeing the patient, the most important questions are asked first. Covers the Glasgow Coma Scale, the systematic thought process behind triage management, and learning points to remember when seeing a particular type of neurosurgical patient. Summarizes evidence-based standards of care and key literature. An excellent learning tool for neurosurgery and neurology residents as well as neurosurgery fellows, registered nurses, neurocritical care health care providers, clinical fellows, physician assistants, clinical nurse specialists, nurse practitioners, travel nurses, advance practice practitioners, emergency room providers, and medical students rotating in neurology, neurosurgery, and neurosciences.
