Mesenchymal Stem Cell Therapy Excerpt :

Over the past decade, significant efforts have been made to develop stem cell-based therapies for difficult to treat diseases. Multipotent mesenchymal stromal cells, also referred to as mesenchymal stem cells (MSCs), appear to hold great promise in regards to a regenerative cell-based therapy for the treatment of these diseases. Currently, more than 200 clinical trials are underway worldwide exploring the use of MSCs for the treatment of a wide range of disorders including bone, cartilage and tendon damage, myocardial infarction, graft-versus-host disease, Crohn’s disease, diabetes, multiple sclerosis, critical limb ischemia and many others. MSCs were first identified by Friendenstein and colleagues as an adherent stromal cell population within the bone marrow with the ability to form clonogenic colonies in vitro. In regards to the basic biology associated with MSCs, there has been tremendous progress towards understanding this cell population’s phenotype and function from a range of tissue sources. Despite enormous progress and an overall increased understanding of MSCs at the molecular and cellular level, several critical questions remain to be answered in regards to the use of these cells in therapeutic applications. Clinically, both autologous and allogenic approaches for the transplantation of MSCs are being explored. Several of the processing steps needed for the clinical application of MSCs, including isolation from various tissues, scalable in vitro expansion, cell banking, dose preparation, quality control parameters, delivery methods and numerous others are being extensively studied. Despite a significant number of ongoing clinical trials, none of the current therapeutic approaches have, at this point, become a standard of care treatment. Although exceptionally promising, the clinical translation of MSC-based therapies is still a work in progress. The extensive number of ongoing clinical trials is expected to provide a clearer path forward for the real

Regenerative Medicine for Spine and Joint Pain Excerpt :

Regenerative medicine (RM) is a rapidly expanding topic within orthopedic and spine surgery, sports medicine and rehabilitation medicine. In the last ten years, regenerative medicine has emerged from the fringes as a complement and challenge to evidence-based medicine. Both clinicians and patients alike are eager to be able to offer and receive treatments that don't just surgically replace or clean old joints or inject away inflammation or work as a stop-gap measure. Regenerative medicine encompasses everything from the use of stem cells and platelet-rich plasma (PRP) to prolotherapy, viscosupplementation and beyond. This book will provide healthcare practitioners dealing with spine and joint pain with the most current, up-to-date evidence-based information about which treatments work, which treatments don't, and which are on the horizon as potential game changers. Chapters are arranged in a consistent format and cover the spine, shoulder, elbow, hand and wrist, hip, knee, and foot and ankle, providing a thorough, top-to-bottom approach. A concluding chapter discusses current and future directions and applications of RM over the next decade or two. Timely and forward-thinking, Regenerative Medicine for Spine and Joint Pain will be a concise and practical resource for orthopedists, spine surgeons, sports medicine specialists, physical therapists and rehabilitation specialists, and primary care providers looking to expand their practice.

This book presents the evidence related to the use of injectable biologics to provide faster and better healing for musculoskeletal lesions and conditions. The authors discuss approaches, such as blood derivatives and cell concentrates, applied to lesions of muscles, ligaments, tendons, bones, meniscus and cartilage, as well as osteoarthritis. Chapters are written by some of the most influential opinion leaders in the field, with up-to-date review of the current literature, where the authors explore both the potential and the limitations of these minimally invasive and promising treatments. The first section is devoted to the formulations and rationale for the use of injectable orthobiologics, while the second section reviews current treatment methods applied to specific joints and pathologies – ranging from tendinopathies through non-unions to articular degenerative processes – as well as the results of these treatment approaches. The third section explores future perspectives, such as pluripotent stem cells, gene therapy, and the stimulation of intrinsic stromal cell niches. Appealing to a broad readership, this book will be of interest to both laboratory research scientists and clinicians, including orthopedists, sports physicians, physiatrists, and regenerative medicine experts.

Application of Stem Cells in Canine Osteoarthritis Excerpt :

Stem cell therapy has emerged as a new direction in the field of regenerative medicine. Hundreds of workers throughout the world continue to refine and explore the role of bone marrow and cord blood stem cells for their regular use to solve hematological and immune mediated disorders since many years. Many of them are working to explore the uses of the different types of adult mesenchymal stem cells found in bone marrow and cord blood that could be corrected by replacing cells in the damaged tissues in their own lineages. In recent years stem cell research is being pursued in the hope of achieving major medical breakthroughs. Scientists are striving to create therapies that rebuild or replace damaged cells with tissues grown from stem cells and offer hope to people suffering from cancer, diabetes, cardiovascular disease, spinal-cord injuries, and many other neurodegenerative disorders. Both adult and embryonic stem cells may also provide a route for scientists to develop valuable new methods of drug discovery and testing.

Early Osteoarthritis Excerpt :

Early osteoarthritis is novel topic for orthopedic surgeons and its process begins immediately after joint injury. The mechanical restoration of the joint surface or ligamentous stability is not sufficient to restore the homeostatic environment in the joint, and this leads to osteoarthritis, despite mechanical stability and alignment. This book sheds light on the various mechanisms and systems involved in the gradual decline of the injured joint toward manifest osteoarthritis. Published in collaboration with ISAKOS, this volume appeals to all stakeholders in surgical orthopaedics and sports medicine.

Cartilage Repair and Regeneration Excerpt :

This work is the result of a partnership that began in 2011, when I received for the first time the invitation to be the scientific editor of a book on bone grafting, by the still little publisher known as InTech. Now six years later, InTech has grown and thrived. My respect and warm approval for the quality of the publisher's work only increased. The hyaline cartilage is a tissue that challenges tissue engineering and regenerative medicine because of its avascular nature. In the 11 chapters of this book, the reader will find texts written by researchers working on advanced topics related to basic laboratory research, as well as excellent reviews on the clinical use of currently available therapies.

Cartilage and Osteoarthritis Excerpt :

Osteoarthritis, the most common form of arthritis, imposes a significant burden of suffering on a growing population of the elderly. Even today, its poorly understood pathophysiology limits the discovery of molecular targets for pharmacological intervention and there are few effective medical treatments beyond pain control and surgery. In Cartilage and Osteoarthritis a distinguished panel of researchers, physicians, and opinion leaders in this challenging field describe their updated classical, but still evolving, techniques, as well as many emerging methods that promise to add significantly to our understanding of cartilage metabolism in health and disease. Volume 1: Cellular and Molecular Tools describes proven molecular and cellular techniques for the in vitro study of normal and osteoarthritic cartilage through biochemical, biomolecular, immunological, and physical approaches, with emphasis on the genetic manipulation of cells. The protocols follow the successful Methods in Molecular Medicine"!series format, each one offering step-by-step laboratory instructions, an introduction outlining the principle behind the technique, lists of the necessary equipment and reagents, and tips on troubleshooting and avoiding known pitfalls. A companion volume, Volume 2: Structure and In Vivo Analysis, offers cutting-edge procedures for studies-at the tissue level-of turnover, structure, and functioning in normal and diseased cartilage by invasive and noninvasive means. Comprehensive and up-to-date, the two volumes of Cartilage and Osteoarthritis provide researchers and bench scientists alike with an indispensable collection of readily reproducible protocols for new experiments-from the cellular to the animal level-designed to more clearly describe the pathophysiology of cartilage, as well as to discover novel molecular targets for pharmacological intervention
