Bronchopulmonary Dysplasia An Update An Issue of Clinics in Perinatology Excerpt :

Bronchopulmonary dysplasia (BPD) as a chronic lung disease affecting preterm infants has been recognized for more than 45 years. However, little progress has been made in the prevention and treatment of the disease. BPD continues to be a major morbidity affecting preterm infants. Studies in pre-adolescent children demonstrate that the abnormal lung function from BPD persists well into childhood. Infants with BPD also have an increased risk for adverse neurodevelopmental outcome. Thus BPD imposes a significant burden of adverse health outcomes in preterm infants. The major risk factors for BPD are prematurity, mechanical ventilation, exposure to noxious insults such as oxygen, infections. New research data both in basic science and clinical studies have shed light on the pathogenesis, and possible new treatment or management strategies for the future. In the proposed issue of the Clinics in Perinatology, we would like to comprehensively cover BPD both from a basic science and clinical perspective. Our attempt is to balance new information along with classic topics. The proposed authors for the issue are recognized experts in the topic area/s. We hope that the issue will be of interest to a broad array of readership.

Respiratory Diseases of the Newborn Infant Excerpt :

So much of our lung health throughout life is determined by what happens before birth and when we are born. Managing respiratory problems in newborn infants requires experience and teamwork, an understanding of the background to the problems, and knowledge of the evidence behind the clinical options available. In this Monograph, science, evidence and expertise are brought together in a collection of comprehensive, state-of-the-art reviews that cover: the structure and function of the newborn respiratory system; neonatal lung disease in preterm infants; developmental, structural and functional diseases of the respiratory system; and more. This book will prove a valuable resource for neonatal clinicians, scientists researching the area and adult clinicians managing lung health.

Manual of Neonatal Respiratory Care Excerpt :

Respiratory care is the largest overall component of neonatal intensive care, and the fifth edition of the Manual of Neonatal Respiratory Care is the leading bedside guide for all aspects of respiratory care in the neonatal intensive care unit. Its easy-to-read outline format is simple yet comprehensive and covers all aspects of lung disease in the newborn infant, including embryology, principles of mechanical ventilation, procedures and techniques, monitoring, devices, adjunctive therapies, management of respiratory illness, complications, outcomes, and related issues. The latest edition includes fully revised and updated information, coverage on new equipment and devices, and an expanded authorship to enhance its international appeal. The new edition also features two new co-editors, Dr. Mark Mammel and Dr. Anton Van Kaam, internationally recognized experts in the field who bring a fresh perspective to the manual. Divided into sixteen sections, the book begins with a section on lung development and maldevelopment, specifically covering the development of the respiratory system, malformations, deformations, disorders of the neonatal airway, and developmental lung anomalies. The second section reviews the principles of mechanical ventilation, with coverage on such topics as spontaneous breathing, oxygen therapy, oxygen toxicity, pulmonary mechanics, and ventilator parameters. The third section of the manual outlines procedures and techniques, including neonatal resuscitation, laryngoscopy and endotracheal intubation, and tracheostomy. The following section dives into the monitoring of the ventilated patient, specifically focusing on continuous monitoring techniques, clinical controversies in pulse oximetry, and echocardiography. The next section spotlights noninvasive ventilatory techniques, such as nasal interfaces, humidified high-flow nasal cannula therapy, and sustained inflation. The sixth section of the manual focuses on ventilatory modes and modalities, with

Bronchopulmonary Dysplasia Excerpt :

This book provides a comprehensive framework for treatment and management of bronchopulmonary dysplasia. In recent years great strides have been made toward understanding the pathogenesis and clinical aspects of BPD, which is the most common chronic lung disease affecting infants. This one-stop resource is written by leading scientists and clinicians in the field, and chapters discuss the most recent developments in the basic scientific, translational, and clinical characteristics of the disease. Topics such as hyperoxia, pre- and post-natal inflammation, and genetics and biomarkers of BPD are included, as well as non-invasive ventilation techniques, nutrition, and radiology applications from pre-term birth to adulthood. The book closes with an in-depth look at emerging therapeutic options for prevention of BPD. Bronchopulmonary Dysplasia is an essential volume for all neonatologists, pediatric pulmonologists, and scientists interested in developmental disorders of the lung.

Nutritional Care of Preterm Infants Excerpt :

Improved conditions of care for premature infants have led to markedly increased survival rates over the last few decades, particularly in very low and extremely low birth weight infants. Nutritional measures play a central role in the long-term outcome, health and quality of life of these premature infants. In this publication, leading experts from all 5 continents present the most recent evidence and critical analyses of nutrient requirements and the practice of nutritional care (with the focus on very low birth weight infants) to provide guidance for clinical application. After the introductory chapters, covering nutritional needs and research evidence in a more general manner, topics such as amino acids and proteins, lipids, microminerals and vitamins, parenteral and enteral nutrition as well as approaches to various disease conditions are addressed. Due to its focus on critical appraisals and recommendations, this book is of interest not only for the researcher who wants to keep up to date, but also for the clinician faced with premature infants in his practice.

Fetal and Neonatal Lung Development Excerpt :

Lung disease affects more than 600 million people worldwide. While some of these lung diseases have an obvious developmental component, there is growing appreciation that processes and pathways critical for normal lung development are also important for postnatal tissue homeostasis and are dysregulated in lung disease. This book provides an authoritative review of fetal and neonatal lung development and is designed to provide a diverse group of scientists, spanning the basic to clinical research spectrum, with the latest developments on the cellular and molecular mechanisms of normal lung development and injury-repair processes, and how they are dysregulated in disease. The book covers genetics, omics, and systems biology as well as new imaging techniques that are transforming studies of lung development. The reader will learn where the field of lung development has been, where it is presently, and where it is going in order to improve outcomes for patients with common and rare lung diseases.

In this book, you'll learn multiple new aspects of respiratory management of the newborn. For example, ventilator management of infants with unusually severe bronchopulmonary dysplasia and infants with omphalocele is discussed, as well as positioning of endotracheal tube in extremely low birth weight infants, noninvasive respiratory support, utilization of a protocol-driven respiratory management, and more. This book includes a chapter on noninvasive respiratory function monitoring during chest compression, analyzing the efficacy and quality of chest compression and exhaled carbon dioxide. It also provides an overview on new trends in the management of fetal and transitioning lungs in infants delivered prematurely. Lastly, the book includes a chapter on neonatal encephalopathy treated with hypothermia along with mechanical ventilation. The interaction of cooling with respiration and the strategies to optimize oxygenation and ventilation in asphyxiated newborns are discussed.
